Acute myeloid leukemia

• Oral: 35.4 mg once daily on days 8 to 21 of induction and days 6 to 19 of consolidation chemotherapy, up to a maximum of 6 courses of therapy (2 induction cycles and up to 4 consolidative cycles). Followed by maintenance dosing starting at 26.5 mg once daily continuously for up to 36 cycles.
  
  • If after two weeks, QTcF is less than or equal to 450 ms, dose should be escalated to 53 mg once daily.
  • Given in combination with chemotherapy for induction and consolidation

Available as 17.5 mg and 26.5 mg tablets. Tablets should NOT be chewed or crushed. Store at room temperature.

FLT3 Inhibitor

Take with or without food

If a dose is missed, patients should be instructed to take missed dose as soon as possible on same day. Patients should NOT take two doses in one day.

Common: Febrile neutropenia, Anemia, Thrombocytopenia, Diarrhea, Nausea, Vomiting, Abdominal pain, Headache, Edema, Infection, Sepsis, Anorexia.

Less Common: Cardiac failure, Myocardial infarction, QT interval prolongation, LFT elevation, Epistaxis.

Baseline before treatment: CBC & differential, electrolytes, liver function tests, serum creatinine, ECG.

During treatment: CBC & differential, electrolytes, liver function tests, serum creatinine as clinically indicated. Weekly ECG’s should be performed during treatment.

• May increase the risk for QT interval prolongation. Avoid concurrent therapy with drugs associated with QTc prolongation and/or Torsades de pointes, if possible. If unavoidable, monitor for QT prolongation or cardiac arrhythmias.
• This drug can impact or be impacted by metabolic enzymes for absorption and excretion. Many drugs can also affect or be affected by these enzymes, so please consult with pharmacy to assess for interactions.
• Avoid concomitant administration with strong CYP3A4 inducers or inhibitors. If concomitant administration with an inhibitor cannot be avoided, dose reduce quizartinib as per product monograph.
• Current drug interaction databases should be consulted for more information.

• Avoid in pregnancy and lactation.
• Cases of QT interval prolongation, Torsades de pointes, cardiac arrest, and sudden death have been reported in patients taking quizartinib. ECGs should be completed, and electrolyte abnormalities corrected prior to initiation.
